The location that the template file is placed in is most important. Download the file from www.tinyurl.com/nfts-information. From this folder navigate to Safety\New Risk Assessment, where you will find two versions of NFTS AutoRA Form, one for Windows and one for Mac.
The Mac version is called NFTS AutoRA FormmasterMacver1.1.dotm. It is important to realise that this is what's known as a template file and for the Risk Assessment form to work it must always originally be opened from the template file. Trying to reuse an already filled-in form usually doesn't work.
Place the .dotm file in this location on your Mac:
/Users/<username>/Library/Group Containers/UBF8T346G9.Office/User Content/Templates
<username> refers to the name that you use to login to your Mac. When the file is placed in this location it should then appear in the New Documents screen that appears when you start Word.
